Wellbeing Group Referrals FAQ/Fact Sheets
- Q1. I’m not a medical doctor, but I am a registered health professional. Can I make a referral to Hearts & Minds wellbeing courses/groups/workshops?
- A1. Yes, we accept referrals from a range of different professionals who work in healthcare, including registered nurses, counsellors, psychologists, social workers, and occupational therapists.
- If you are still unsure if your profession is able to refer, please call us on 09 441 8989 to discuss this.
- Q2. How can I make a referral to Hearts & Minds wellbeing courses/groups/workshops?
- A2. You can make a referral to Hearts & Minds wellbeing courses/groups/workshops by filling out our referral form (this can be downloaded from our website), and sending it back to us through one of the following methods:

- Q3. What are the criteria for referral?
- A3. The criteria for clients to be able to access our WDHB funded wellbeing courses/groups/workshops are, that clients:
- Have given their consent for the referral.
- Are at least 18 years old.
- Live, or access health services, within the Waitemata District Health Board catchment area.
- Be experiencing mild to moderate mental health concerns only (e.g. anxiety, depression, stress)
- Q4. What does ‘mild to moderate’ mental health concerns mean?
- A4. Mild to moderate refers to clients whose wellbeing is affected by their mental health, however they are still stable in themselves and able to act independently in their everyday lives, and are not at risk of harming themselves or others.
- Q5. Do I have to use the Hearts & Minds referral form, or can I use the same referral process as we use for other services?
- A5. The Hearts & Minds referral form is our preferred form as it provides us with the essential information we require. If you wish to use your own referral form, please ensure all our questions are addressed in your referral, otherwise we will need to contact you to provide the appropriate information.
- Q6. Can I register my client/patient for a Wellbeing Support Course/Group/Workshop?
- A6. No, however you may recommend a particular wellbeing course/group/workshop for us to consider when we are processing their referral.
